COMMUNITY RESOURCES
AGING AND LONG TERM CARE

Senior Information and Referral: an access point for receiving information about services for the elderly. Services include: providing information, service referral, assistance with accessing services, advocacy, and screening to determine appropriate referrals or the need for case management.

Case Management: assists vulnerable seniors in obtaining and using support services. Services include a comprehensive needs assessment, service plan development, management and monitoring.

Respite Care Referral: provides relief for families and other unpaid caregivers of vulnerable adults 18 and over. A family can receive up to 150 hours a year in Respite Care and fees are determined on a sliding scale.

Personal Care/COPES Nurse Oversight: Registered Nurses evaluate service plans in relation to any changes in condition or environment, notify Case Manager of any changes, monitor the quality of care by the paid caregiver and provide task specific and classroom training.

HOPE: a joint project, administered by the Richland, Kennewick and Pasco Housing Authorities, to provide rental assistance and supportive services to low income elderly. Persons must be 62 or older, low income and need assistance with personal care and household management.

SCSA Personal Care Services: Provides personal care services to people age 60 and over who are not eligible for like services provided through another source. Services are based on a sliding fee scale.

Farmworker Outreach Program: Outreach program to elderly farmworkers to educate them regarding the community resources available to the elderly.

IN HOME MEDICAL CARE

Provides integrated homecare services for breast and other cancers, respiratory disorders, immune deficiencies, cardiovascular disease, infectious diseases and pregnancy complications. Provides infusion therapy and integrated respiratory care services. Medicare, Medicaid, Insurance, Private Pay. .

Provides skilled, intermittent nursing and therapy services for homebound and recuperating patients. Services include patient care and education, wound care, comprehensive infusion therapy, diabetes care, rehabilitation, maternal/child care, and medical social work. Clinical Specialists include: Registered Nurses, Physical therapy, Occupational therapy, Speech Therapy, Medical Social Worker, Enterostomal Therapist, Certified Wound Nurse, Certified Infusion Nurse, Certified Diabetic Educator, Home Health Aides. Home Health Community (HHC-Respiratory Care, Inc) 586-4275

A team of accredited respiratory care practitioners who instruct, encourage and review therapy plans. Specialized service includes oxygen therapy, respiratory rehabilitation, children's asthma workshops, pulmonary aids and monthly patient assessments. Community education programs on a range of lung and heart topics such as: asthma, smoking cessation and pulmonary wellness are offered. A large selection of quality respiratory home care products are also available. Medicare, Medicaid, pre-authorized case management medical claims.

Hospice 783-7416

The Hospice team of physicians, nurses, home health aides, volunteers, spiritual counselors and social workers work together to provide compassionate, home-centered care for terminally ill (six months or less) people and their families. Provides hands on care for dying people and their loved ones, provides education regarding pain management and the disease process, supports the family emotionally, and provides special bereavement care for the family up to 13 months. Hospice House has six beds for those people unable to stay in their home. Medicare, Medicaid, private insurance.

Kennewick General Hospital Home Health Care 582-2273

In home services include Registered Nurses, Licensed Practical Nurses and Nurses Aides, up to 24 hour coverage in the privacy and comfort of home or short visits to change dressings and catheters, administer injections and making medical assessments. Richland Knights of Columbus Community Hospital Equipment Lend Program (HELP). They can provide hospital type beds, over the bed tables, lift chairs, wheel chairs, bath and shower chairs and portable bathroom equipment in the home. They will deliver and set it up for you. There is no charge and one does not have to be a Member of the Knights of Columbus to be able to receive the equipment.

IN HOME PERSONAL CARE

Home Care Services 735-7840

A community program of the Benton-Franklin Council on Aging providing services 24 hours a day, 7 days a week with no minimum hour requirements. Services include: meal preparation, bathing, dressing, shopping, transportation, household assistance, personal hygiene, body care, eating assistance, toileting, ambulation, protective supervision, self-medication assistance, transfer/positioning, and family care. State programs include COPES, Respite, Chore, and HOPE as well as private pay.

Total Care, Inc. 783-3600

A professional team of RN's, LPN's, NAC's, and NAR's providing care in the home, hospital or nursing home. Home Health Aides provide assistance with bathing, dressing, oral care, exercise, meal preparation, laundry, light housekeeping, transportation and companionship. There is a 3 hour minimum time limit and services are available 24 hours a day, 7 days a week.

State programs include COPES and Respite, also insurance and private pay.

A register of people who are available to do personal care in the home. ALTC does a background check before listing the individual on the register. The patient/family is responsible for interviewing, calling references and hiring the individual. The patient/family is responsible for payment and any appropriate state and federal payroll taxes.
 

NUTRITION

Senior Nutrition Program 783-2660 A community program administered by the Benton-Franklin Council on Aging serving congregate meals, home delivered meals and frozen dinners. Nutritious lunches are served to people 60+ at nine congregate meal sites in Benton and Franklin Counties.

. Home delivered meals are available for seniors (60+) who require assistance to leave home, have a disability or illness which makes the preparation of the meals difficult and has no one in the home to assist with meal preparation. A home visit is made to determine eligibility. A private pay program is available for those people who do not meet the above requirements.

Special meals are available for the liberal diabetic and the mild, sodium-restricted diet. Donations of $2.25 are requested but no eligible senior is ever refused meals due to an inability to donate. A week's supply of frozen meals are available for persons living outside the delivery area. An Ensure delivery program is also offered to seniors who are underweight or are unable to consume adequate nutrients by food alone due to medical treatment or illness.

American Cancer Society 783-5108

Provides a credit of $200 per year for Ensure for patients with cancer. A prescription is required from the attending physician.

TRANSPORTATION

Dial-A-Ride 735-0160

A service offered by the Ben Franklin Transit District. One must send in an application for approval. Rides must be requested 24 hours in advance. Service is available weekdays from 6 a.m. to 7 p.m. and Saturday from 8 a.m. to 7 p.m. Rides are 50c and ticket books are available. Night service is available Monday-Saturday from Ben Franklin Transit Trans+Plus from 7 p.m. to 11 p.m. There is no service on Sunday. Service is available in Pasco, Kennewick, Richland, and part of West Richland. Daytime Dial-A Ride service is available in Finley for $1.25.

American Medical Response 582-6418

Emergency (dial 911), taxi and non-emergency local and long-distance (Seattle, Spokane) transportation in ambulances, van and sedans. Twenty-four hour paramedic service available. Medicare pays for ambulance but not for ambulatory, non-emergency van or taxi. Accepts Medicare, Medicaid, Healthy Options, Basic Health, private insurance or private pay.

Medicaid transportation for non-emergency medical appointments. A valid medical coupon is necessary and reservations should be made 48 hours in advance.
 
 

PERSONAL EMERGENCY RESPONSE SYSTEM

Checkline Telephone reassurance 943-9017

A telephone reassurance for seniors and other people who can benefit from a regular, daily phone check on their well-being or as a medication reminder. Each new client is visited by a Check Line staff member to obtain pertinent information (address, person to contact in case of emergency, back-up people, etc.). There is no charge for this service. This is a service of Contact Dial HELPline.

Lifeline of Eastern Washington 1-800-249-5433

Emergency response system with two way voice communication use of a waterproof button worn on a pendant or on the wrist. There is no equipment to purchase or long term commitments. Within a minute of pressing the button, a Lifeline monitor responds to you. If you say you need assistance, your monitor will send appropriate help. Extra features include an Inactivity Alarm that lets a person establish a period of time which, if Lifeline has not heard from them, the communicator will signal for immediate assistance.

If you need help for any reason, push the LifeGuard Personal Help Button to activate the Home Communicator and notify the LifeGuard Emergency Center. Certified LifeGuard Monitoring Staff will immediately call your home to assist you. If you cannot be reached, your responders or emergency services will be sent to help you. The Monitoring staff will continue to monitor the situation until help arrives, the communicator has been reset and they have talked with the people assisting you.

MENTAL HEALTH SERVICES

A voluntary agency providing 24-hour service in telephone listening and caring, crisis intervention, information and referral.

Benton/Franklin Crisis Response (24 hours a day) 783-0500

Mental health crisis services available twenty-four hours a day, seven days a week. Services include: crisis line including information and referral, community crisis outreach and stabilization services, crisis intervention services including face-to-face screening and referral, brief counseling, implementation of the Involuntary Treatment Act (ITA), emergency assistance with psychiatric medications, community education regarding mental health issues, access to adult and child mental health crisis residential supports, liaison with State and local psychiatric hospitals for treatment and discharge planning and liaison with the Substance Abuse ITA unit including treatment planning for Mentally Ill, Chemical Abusing (MICA) clients.

Provides hospitalization and partial hospitalization for adults, adolescents and children, out patient therapy, child, adult and family counseling. A geriatric registered nurse is on staff for consultation to individuals, families and the community.

Nueva Esperanza Counseling Center 545-6506

Senior Peer Counseling Program: a new outreach program specifically designed to meet the needs of the elderly. Volunteers (ages 55+) are trained to provide one-to-one supportive counseling to senior citizens who are confined to their home or are uncomfortable in a traditional mental health setting. Volunteers provide support, reassurance and serve as a resource advocate to vulnerable individuals who are alone, isolated and in need of some type of intervention.

Sunderland Family Treatment Services 736-0704

Confidential care management provides help with consultation by geriatric mental health specialists and psychiatric nurses, counseling for grief, stress, anxiety and relationship problems, assessment of mental status, medication evaluation, support of caregiver and referral to community resources. Patients are seen in the office or in their home. Medicare, Medicaid, insurance, private pay (sliding scale is available).

Tri-Cities Chaplaincy 783-7416

Provides grief consultation for adults and children and various support groups including CARES (Columbia AIDS Relief, Education and Support) for persons, families and friends of people affected by or infected with HIV+ or AIDS.

A free service membership program for people over 50 years old offered by Kadlec Medical Center. Benefits include: a specially trained Care Coordinator available to assist with community resource planning, monthly programs on a variety of topics on healthcare issues, discounts for medically related services and products as well as for travel, quarterly subscription to The ElderMed Magazine, and assistance with completion of insurance and Medicare Supplement claims, information regarding health insurance policies and a person to act as an intermediary between you and health care professionals.

A non-profit agency providing education, information and support to persons with neurological disorders and to their families, caregivers and friends. A library is available with books, video and audio tapes, brochures, self-help newsletters and pamphlets. A handicapped equipment loan closet is available. Our Lady of Lourdes Health Center--Senior Services 543-2412 Integrated inpatient services include Emergency services, acute medical/surgical care, chemical dependancy, mental health, skilled nursing unit and rehabilitation units. Nurse case management is available for high risk patients of Our Lady of Lourdes throughout the continuum of care (in patient and community follow-up). Outpatient services include ambulatory surgery, diabetic education, nutrition counseling, mental health assessment and treatment, chemical dependency treatment, rehabilitation therapy, pain clinic, diagnostic services and home health care.

Health promotion and wellness programs include the "healthy living/successful aging" series presented at retirement centers and other sites throughout the community, medication consultation by pharmacists, community health screens as requested, and support groups.

LEISURE

Elder Day Care 586-9344

A not-for profit agency providing adult day care and caregiver respite for people with physical, mental and demential limitations. Program activities include community education, meals, transportation, nursing, physical therapy and therapeutic activities. A quality program is offered which stimulates the mind and body and nurtures companionship in a loving and respectful environment. Individuals must be 18 years and older.

The Senior Center offers a variety of activities, classes, day trips and extended tours.. There is a computer lab, daily activities and monthly special events. Daily programs could include: crafts, card games, step or low impact aerobics, Bible class, etc. Monthly or biweekly activities include: blood pressure checks, assistance with health insurance claims, hearing clinic and hearing aid testing, eye glass frame adjustments, dances. Lunch is served Monday through Friday from 11:30 am to 12:30 pm through the Senior Nutrition Program as well as a soup and salad bar. Call for copy of monthly newsletter as activities change monthly.

The Pasco Senior Center is open Monday thru Friday, 8 am to 5 pm. There is a computer lab available for personal use as well as classes. Each day has a variety of activities including such things as: pool, walking, board and card games, crafts, sewing and quilting. Bimonthly and monthly activities include: free blood pressure checks, Ask A Doc, birthday potlucks, and movie day. The Senior Center also offers a health screening program and the preventive foot health care program. Nutritious meals are served Monday thru Friday from noon until 1pm. Please call for schedule of current activities.

Richland 943-7377 The City of Richland welcomes all persons 50 years old and older to participate and enjoy a variety of social and recreational activities at the Richland Senior Center. On-going activities include: an indoor walking program, 8-ball pool, duplicate bridge, scrabble, bingo, pinochle, S.H.I.B.A. volunteers, party bridge, cribbage, and greeting card making. Special monthly events, educational programs and day trips are also offered. The Senior Nutrition program is offered Monday through Friday at noon. Volunteers providing needed services to elderly citizens who need companionship type services to maintain their own homes. Companions help with arranging transportation, assist with completing Medicare or Medicaid applications, help plan meals, assist with home management, read to their clients, take them shopping and write letters for them.

Parent Care, offered by Crawford Care Management Services, provides professional assistance to seniors who wish to live independently. Upon referral, a thorough on-site assessment is completed to determine current and future needs. A confidential, detailed analysis is completed along with a care plan and recommendations for intervening with cost-effective services and resources. Care Management services include: crisis intervention, in-home assistance arrangements, on-site assessments, daily monitoring phone calls, nutrition and dietary management, location and assistance with alternate living arrangements, assistance with Medicaid applications, Nursing Home Advocacy Program, and grief/crisis counseling. MISCELLANEOUS

Disabled Parking Permits

Applications may be obtained from the auditor's office at 1016 Jadwin (Richland) 943-3117; 5600 W. Canal Place (Kennewick) 736-2727, or the Pasco Court House, 1015 N.5th (Pasco) 545-3533.

A permanently disabled person can choose one or two parking placards, or one set of license plates or one placard and one set of plates. Temporarily disabled persons can only receive one placard. Placards are free and are hung on the rear-view mirror when parked; license plates are $9.00 and are installed on the vehicle. The applicant must sign and give a daytime phone number and their Washington driver's license number or Washington I.D. number. The application is then completed by a licensed physician and returned to the Auditor's office. Upon approval, the placard and/or plates are mailed to the applicants. Placards can be used on any vehicle while plates are not transferable.

A licensed physician must attest to at least one of the following conditions:

Cannot walk 200 feet without stopping to rest

Walking is limited by arthritic, neurological or orthopedic condition

Cannot walk without use of a brace, cane, another person, prosthetic device, wheelchair, etc.

Uses portable oxygen

Restricted by lung disease to a specified extent

Limited physical activity from cardiac disease

Walking disability resulting from auto emissions

Volunteer Chore Service 943-8349

A network of volunteers who help low-income people with functional disabilities, 18 and older. Service can include: housework, yard care, cooking, wood provision, transportation, minor home repairs, medication reminder, communications, laundry, shopping/errands, moving assistance and monitoring. Special training is provided for protective supervision and limited personal care. Most clients have an income of less than $600. Unusual expenses (such as medical) will be considered for individuals with incomes of $800 ($1200 couples). Assistance is based on availability of volunteers and may vary greatly depending on area and type of request.

Adult family homes are licensed by the State of Washington to care for two to six residents. All homes promote a high standard of personal independence to ensure the highest quality of life. Services include: private or semi-private rooms, family style meals, snacks, laundry and housekeeping, supervision of medication, companionship and emotional support, personalized care, and assistance with daily living activities. Assisted living apartments feature an emergency response system as well as customized services base on personal level of care. These services can include dressing and bathing assistance, medication reminders, and nutritional supervision.

A speciality care 56-bed facility for people with Alzheimer's disease and other dementias. All services are provided under the direct supervision of licensed, professional nursing personnel and all staff members are specifically trained in the special needs of Alzheimer's residents. There are secured perimeters, including a beautifully landscaped enclosed courtyard and secured outside walking area. All medications are kept at a minimum in a restraint-free environment. A unique residence specially designed to help those with Alzheimer's disease or similar dementia to live dignified life-style within a secure and supportive environment. Each resident is encouraged to function at the highest level possible--for as long as possible--doing the activities of daily living with dignity and security. Residents enjoy access to a fenced outdoor courtyard. Staff are specially trained in Alzheimer's Disease and other dementias.

Twenty-two bed specialized Alzheimer's Unit at Lifecare in Kennewick. All staff have been specially trained. Outreach program provides house calls by Registered Nurse to educate, support, and assist families caring for patients with Alzheimer's disease and other dementia in the home.

The Lourdes Restorative Care Unit provides skilled nursing and rehabilitation for up to 100 days to patients who need immediate post hospital care for strengthening and improvement of functions prior to discharge. The unit provides acute and subacute services (ie: IV therapy, wound care, respiratory therapy, etc.).

Home and Community Services determines financial and functional eligibility for COPES, Medicaid Personal Care, CHORE, Residential Services and Institutional facilities. Once a person has been determined eligible for COPES, the case management for in-home clients is transferred to Aging and Long-term Care case managers in Benton and Franklin counties. Persons determined eligible for residential care or institutional services are managed by the Home and Community case workers.

In addition, Home and Community Services provide Adult Protective Services (APS) for frail elderly or vulnerable adults.

There are both income and asset financial eligibility requirements for State programs. They can be complex and new guidelines are determined every year. For complex situations, one should contact Home and Community Services or an attorney. Briefly, financial requirements are:

Single Person: ASSETS:: $2000 in non-exempt resources (money in the bank, CD's, etc.)

Exempt income includes: home if applicant states he/she intends to return home, household furnishing and personal effects, automobile with a blue book value below $4,500, a separate and designated burial fund of $1,500, life insurance with face value of less than $1,500, resources that cannot be converted to cash within 20 days.

INCOME: Cannot exceed $1,452 as of January 1, 1997

Married Persons: Eligibility rules for married couples apply when one spouse is in a nursing home or on COPES. If both spouses are in a nursing home or on COPES, they will be treated as though they were single and the rules for single persons apply.

ASSETS: For married couples when one spouse seeks COPES or Medicaid nursing home coverage, all resources of both spouses are considered in determining eligibility. The community spouse is allowed $79,020 in non-exempt resources in addition to the $2,000 in non-exempt resources allowed the institutionalized or COPES spouse. All but $2,000 must be transferred into the name of the community spouse before the first regularly scheduled eligibility review, which is usually 12 months after initial eligibility is determined. In addition, each spouse is allowed to have a $1,500 burial fund. The home is exempt if the community spouse resides in the house and one car per couple is exempt regardless of value.

INCOME: Spouse receiving COPES: not to exceed $1,452 (1997)

There is no income limit for the spouse not receiving COPES

Income Participation: Depending on income level, there can be a participation cost to the patient. COPES allows the first $645 (1997) of income, if client is in their own home, to be retained for living expenses. There is no cost participation required of a couple where one spouse is on COPES unless their combined income exceeds $1,807 (1997).

Medicaid Estate Recovery: Under federal statute, the Department of Social and Health Services has a statutory lien against the estate of a Medicaid nursing home or COPES recipient for most of the cost of medical care paid for by Medicaid after the recipient turns 55. Medicaid estate recovery cannot proceed as long as there is a surviving spouse or a surviving child who is under 21 or blind or disabled, and recovery is not authorized against the estate of such a spouse or child.

Program Eligibility:

COPES: Community Options Program Entry System

COPES is a program designed to assist persons to remain in their home or the least restrictive setting who would otherwise require care in a nursing facility. Service eligibility includes: a person requiring substantial or total assistance with two or more of the following self-care tasks and has unmet needs: eating, toileting, ambulation, transfer, body care or bathing. A person with cognitive supervision needs and requires substantial or total assistance with one of the tasks above.

Medicaid Personal Care: A program for single persons with a monthly income less than $532 (1997) per month (they are not eligible for COPES) who need help with at least one personal care task because of a medically-related condition.

CHORE: Available on a limited basis for persons not eligible for COPES or Medicaid Personal Care. Asset/Resource limits are higher than COPES. The person must be at risk of placement in a long-term care facility, needs help with at least one personal care task, and has no one available to provide the needed help. .
